Pharmaceuticals are essential technologies in healthcare and represent a considerable portion of healthcare expenditures in countries worldwide. The budgetary perspective of pharmaceutical spending is essential for analyzing the weight of these expenses in relation to the resources allocated for health financing each year. However, in Brazil, in recent years, the records of these expenses made by states, the Federal District, and municipalities in national budgetary systems have declined in quality, primarily due to the lack of complete information. Thus, with the aim of, among other objectives, improving the estimation of these entities' spending on drugs, the Pharmaceutical Service Research in the Unified Health System (SUS) was conducted. This paper analyzes pharmaceutical expenditure from 2019 to 2023 for the municipalities and states participating in this research. The results highlight that most of the total pharmaceutical expenditure (TPE), both in states and municipalities, was financed through local resources, with a reduction in federal participation in funding this expense over the period. Furthermore, the TPE in absolute values, as a percentage of total health expenditure (THE), and per capita values, increased between 2019 and 2023 in both cases. Municipalities with populations of up to 5,000 inhabitants had a higher TPE per capita compared to other municipal groups categorized by population size. It is concluded that, although the research results cannot be generalized to all municipalities and states in the country, they point to the need for a re-evaluation of the pharmaceuticals financing and the management of drugs procurement across the three levels of government in SUS.
