Abstract: 
We examine the causal impact of hospital competition on quality of care for AMI, stroke, and joint replacements. To address endogeneity, we use a two-pronged instrumental variable approach: predicting market shares via demand models and instrumenting market structure using local election margins. High competition significantly improves health outcomes for AMI, driven by medical staff intensity and technological infrastructure availability. We find no effect for stroke and joint replacements.
